Pharma company clarifies on effects of Gardasil vaccine

MSD Pharmaceuticals Private Limited, the company that markets Gardasil in India, has said that the vaccine can help prevent cervical, vulvar and vaginal cancers and genital warts caused by the HPV (human papilloma virus) types 6, 11, 16 and 18, as also some protection against 10 additional cervical cancers.

‘Ten years of research'
“Gardasil is the result of over 10 years of research and development. As part of the rigorous scientific vaccine clinical development programme, clinical trials evaluating the efficacy and safety of the vaccine have included more than 25,000 women from 33 countries from around the world,” a statement issued by the company said in response to criticism of its clinical trials in India reportedly carried out in an unethical manner.

In India, the Indian Academy of Paediatrics Committee on Immunization (IAPCOI) stated that the HPV vaccine was of public health importance and recommended giving it prior to sexual debut.

U.S. Recommendations
In 2006, the United States Centers for Disease Control and Prevention's (CDC) Advisory Committee on Immunization Practices recommended that girls and women between 11-26 years be vaccinated with an HPV vaccine such as Gardasil, the statement said.
While it is difficult to determine the exact number of doses administered, since its launch in 2006, more than 55 million doses of the anti-cancer vaccine have been distributed worldwide as part of the global fight against HPV diseases.

Robust surveillance
The statement claimed that MSD has established a robust surveillance programme to monitor the long-term safety, efficacy and duration of protection of people vaccinated with Gardasil.

“MSD monitors vaccine safety by conducting analyses of adverse events reported to MSD, and we share these adverse event analyses with regulatory and medical authorities around the world.

Preliminary studies
“Preliminary studies conducted to date have encouragingly demonstrated that there is no waning of protective immunity at five years following vaccination.
“MSD is conducting long-term follow-up studies to confirm whether immunity to HPV disease is long-lasting,” the statement said.

While no vaccine or medicine is completely without risk, leading international health organisations throughout the world — including the Drug Controller-General of India, World Health Organisation (WHO), the CDC, Health Canada, the European Medicines Agency (EMEA), and the Australia Therapeutic Goods Administration (TGA), among others — have reviewed all of the safety information available to them about Gardasil and continue to recommend its use, the statement added.

New technology can identify counterfeit drugs in minutes

A leading UK expert in analyzing pharmaceutical compounds has demonstrated that U.S.-based technology can identify fake medicines in minutes.

Traditional technology, based on large, laboratory-based methods, takes hours and sometimes days of intensive work. Presenting his research at the British Pharmaceutical Conference (BPC) in Manchester, Professor Tony Moffat said: "This new technology allows analysis from a scraping rather than from a whole crushed tablet (which saves time and effort) and can identify counterfeits in real time - two important benefits over existing technology - that offers wholesalers, regulators and governments the opportunity to up-scale their efforts to detect fake drugs that are increasingly entering the supply chain." Professor Moffat is Head of the Centre for Pharmaceutical Analysis at the University of London's School of Pharmacy.

Identification of a fake drug by visual examination is almost impossible. Increasingly, international and national regulators, law enforcement personnel, pharmaceutical companies and wholesalers are looking for ways of reducing counterfeits entering the supply chain. Recent examples of counterfeits entering the UK supply chain include Plavix (to prevent blood clotting), Casodex (for prostate cancer) and Zyprexa (for psychosis).

The technology, called Direct Analysis in Real Time (DART), produces an almost instantaneous reading of the chemical composition of the outer coating of a tablet - or of its core composition - based on a scraping of the tablet. Traditional analytical techniques require tablets to be crushed to a powder and processed before they can be assessed.

To test the potential of DART technology, Professor Moffat and his team analysed authentic Cialis (erectile dysfunction treatment pill) tablets which contain an active ingredient called tadalafil, bought from pharmacies in London, and compared them with known counterfeit Cialis tablets provided by the Korean Food and Drug Administration. The film coating of the authentic drugs contained a signature chemical that was detected by DART in all instances.

DART correctly excluded all the counterfeits because of this signature chemical. Moreover, when a scraping of the tablets was used to examine the core composition of the tablets, DART revealed that none of the counterfeits contained tadalafil. Instead, they contained the active ingredient of Viagra, sildenafil. Professor Moffat said: "DART clearly differentiated the authentic from the counterfeit preparations within a few minutes. This technique is minimally destructive and gives accurate and quick readings. There is great potential for this technology to be used more widely in efforts to reduce the market fake medicines."

Men bored with marital sex after five years

ABOUT 80% of married men face erectile dysfunction five years into their marriage because of having a single sex partner, China Press reported yesterday.

It quoted Singapore's Dr Yu Wei Siang, who is otherwise known as Doctor Love, as saying that 50% of men faced the problem with their sex partner after being married for between three and five years and 80% of them after being married for five years.

The paper said it was normal for a married couple to experience low libido after having the same partner for a long period of time.

I was watching the boob tube last night and saw a commercial for ED, that thing ole Bob Dole made a couple of million smackeroos on. It paid better than being a Senator fer shur. Senator is worth maybe 200 large, but ex Senator with ED is boomtown buckalucka's.

These Erectile Dysfunction commercials are really prevalent in the adult hour, 10-11 EST. It's the same time they run all the female commercials. Thank god for the remote.

This commercial for Cialis was one I had never seen before, and it had an interesting twist. There is a black couple who are walking down the street whispering sweet nothings or dirty somethings to each other, and "Dude" has obviously taken his pill and the blood is rushing in all the right places.

Then they are spotted by a white couple they know, who invite them into a juice joint to have a drink. They look at each other and sigh, and they have to be thinking, "White folks can really mess up your day."

But they acquiesce, and go in. They are all shown at a table laughing and talking, but "Dude" is okay because Cialis lasts for 36 hours. That's enough time for those pesky white folks, and a horizontal bop.

Gene therapy 'hope' on impotence

US scientists say they are closer to creating a gene therapy treatment for erectile dysfunction.
Human and animal trials suggest this could offer an alternative to current treatments for some patients, the American Society of Gene Therapy heard. Researchers say gene therapy could be effective far longer than pills used just before sex, improving spontaneity.
The human trials involved injections into the penis and some experts queried whether men would choose this option.

New approach

Erectile dysfunction (ED), often referred to as impotence, is thought to affect about one in every 10 men in the UK. ED collectively means an inability to get a good enough erection to achieve satisfactory intercourse, but it varies in severity. There are many causes, and many effective treatments, including drugs like the three licensed in the UK - Viagra, Cialis and Levitra.
But not all types of ED respond well to medication - for example, ED caused by nerve damage following prostate cancer surgery. US researchers are hopeful that for these patients, and those who experience side-effects with medication, gene therapy may be a good alternative.

Nerve damage

A University of Pittsburgh team, led by Dr Joseph Glorioso, tested an experimental gene therapy in rats with ED caused by nerve damage. The gene therapy used comprised the herpes simplex virus as a carrier and either a gene called GDNF, or one called neurturin, which both help promote nerve growth. Rats treated with the gene therapy showed significant recovery and were able to regain normal penile function after four weeks.

Meanwhile, scientists at Wake Forest University, with the help of Dr Arnold Melman from the Albert Einstein College of Medicine, have been conducting the first human study of gene therapy for Erectile Dysfunction.

Tests on 11 men with ED showed promising results. The treatment was well-tolerated, with few side effects, despite its delivery via an injection into the penis. Although the primary goal of the study was to determine its safety, it also showed the therapy improved erectile function in some of the men.

Dr Melman said: "This is an exciting field of research because current treatments for men with erectile dysfunction, whether pills or minimally invasive therapies, must be used 'on demand', thereby reducing the spontaneity of the sexual act." 'Chronic disease'
The Wake Forest therapy works by inserting small pieces of DNA into cells to trigger the production of proteins which, in turn, help smooth muscle cells relax. Relaxing the smooth muscle in the penis allows it to fill with blood and achieve an erection.

Dr Geoff Hackett, president of the British Society for Sexual Medicine, said the gene therapy might be appealing to some men for whom other treatments had failed, but predicted many men would be reluctant to have a shot in the penis. He stressed that many men with ED also had underlying medical disease, such as high blood pressure, raised cholesterol or diabetes, and that this should also be treated.

And there is a good argument for treating ED as a chronic disease too, he said. "Maybe men with ED should be taking a pill for it every day. If you take smaller amounts regularly, the side-effects are less," he explained.

Scientists are a step closer to curing erectile dysfunction following prostrate or bladder surgery - after successfully carrying out tests on rats.

The technique uses gene therapy to stimulate the growth of a crucial nerve during surgery. Scientists are now in the early stages of setting up a trial of the treatment in human patients.
Men undergoing surgery on their bladder or to remove a tumour in their prostate frequently suffer damage to a crucial nerve, which affects their ability to produce an erection. This can take months or weeks to cure, if at all, and drug treatment for erectile dysfunction such as Viagra are not effective. Prostate cancer is the most common cancer in men and there are nearly 32,000 new cases in the UK each year.

Now Joseph Glorioso, of the University of Pittsburgh school of medicine and his colleagues have shown that, in laboratory tests on rats, it is possible to solve the problem using gene therapy. Their approach involves treating the nerve with genes that code for nerve growth factors. These prompt the rapid repair of the nerve and so lead to recovery of sexual function. "The treatment is provided right where it is needed," said Dr Glorioso.

The genes are delivered inside a modified herpes virus which can enter cells and deliver DNA to the nucleus, but cannot replicate because some of its own genetic material has been removed. Added to the virus are copies of the nerve growth-promoting genes, so once in the nucleus the modified virus DNA sets about producing these chemicals. The virus is also tweaked so that it cannot invade any cells other than nerve cells.

After four weeks, the rats that received the growth factors showed significant recovery of both the nerve and erectile function. The team reported their research at the annual meeting of the American Society of Gene Therapy.

The Sexual Dysfunction Association estimates that one in 10 men in the UK - around 2.3million - have recurring problems with their ability to get an erection at some point in their lives. A quarter of these are caused by psychological conditions, but around 75 per cent are due to a variety of physical causes including cardiovascular disease and diabetes.

Other researchers are pursuing gene therapy treatments for erectile dysfunction. One study of a treatment called maxi-K gave promising results in initial tests on human patients and has now moved into a more extensive clinical trial. The treatment includes a replacement for a defective gene in muscle cells. Another trial on rats is investigating gene therapy to cure erectile dysfunction brought on by diabetes.
